Free Health Checks for Medical Card and Doctor Visit Card holders
We are participating in a HSE program to offer free health check appointments, including blood tests, to eligible patients who have medical cards or doctor visit cards. You may be eligible for these programs if you have:
High blood pressure
Are a smoker
BMI over 30
High Cholesterol
Mental Illness
Kidney disease
Recent diagnosis of diabetes in pregnancy
Chronic illnesses such as asthma, diabetes, COPD, heart conditions, stroke or mini stroke

Crumlin Family Practice is a GP practice established over 30 years ago caring for the community in Crumlin and surrounding areas.
We are committed to providing the highest quality medical and nursing care for our patients and offer the full range of GP services.
Dr. Áine Fitzpatrick
Dr Fitzpatrick graduated from Trinity College Dublin in 2005. She became a member of the Royal College of Physicians of Ireland in 2008. She completed vocational training in General Practice with the Royal College of Surgeons of Ireland in 2015
Dr. Naoimh Kenny
Dr Kenny graduated from The Royal College of Surgeons of Ireland in 1996. She completed vocational training in General Practice with the Royal College of Surgeons in 2000 and has been working in General Practice since then

If you would like to register as a patient at Crumlin Family practice please complete and return the New Patient Registration form (download here) and the Transfer of Medical Records form (download here) forms.

If you have a medical card please complete and return the Application to change GP (download here) form also. Please return completed forms to us by post or by email (crumlinfamilypractice@protonmail.com).
PLEASE NOTE: If we have space available to join our practice, we will contact you by text or email to confirm that you have been registered. Please note that spaces on our patient panel do fluctuate and this is reviewed weekly.

For clinical questions please book an appointment with our doctors, once you are registered as a patient. Thank you.
Protonmail
Do not use the protonmail email address (crumlinfamilypractice@protonmail.com) for medical queries or sending medical data. It will not be reviewed by Medical staff. Please only use for the specified purposes.
Prescription Requests
Requests for repeat prescriptions for your regular medications must be made in writing please. Requests can either be emailed to us at crumlinfamilypractice@protonmail.com, or dropped into the practice. Please include your name and DOB, and the medication name and dosage on your request. Please allow 3 working days for all prescription requests. Please note that this email address is not appropriate for any clinical questions. Please book a consultation if you have queries about your medication.
Your chemist will provide one weeks supply of any medications prescribed in hospitals or outpatient clinics, to allow time for our doctors to prepare the appropriate medical card prescription for you.
Appointments
Crumlin Family Practice offers appointments for all of our doctor and nurse led clinics. Please contact our reception to book an appointment prior to attending. We do not have a ‘walk in’ clinic but we will always try to facilitate patients who require care at short notice. Please advise our receptionists if you require an urgent appointment.
Disability Access
Crumlin Family Practice is located on the ground floor. It is fully wheelchair accessible and has been granted a Disability Access certificate.
Parking
Patient parking is available free of charge outside our practice.
Obtaining Test Results:
If you are looking to obtain test results please contact the surgery during opening hours 7 days after the test has been performed.
